For care teams
Once a patient goes home, the people doing the work are a spouse and two adult children with a folder of instructions. Postward gives them one shared plan, one medicine log and one daily board, built from your instructions and your warning signs.
Hand families a card at the pre-op class. They arrive at the two-week visit with a dose log, check-ins and their questions written down.
You see these patients two or three times a week. The home exercise plan becomes a task the whole family can see and tick off.
Your 30-day episodes depend on what happens at home. We're building a care-team view, under a business associate agreement, with a small number of partners.
Postward is in private testing with families. We have no hospital customers yet and make no outcome claims. We're looking for two or three practices or clinics in New York to shape the care-team side with us.
Postward doesn't triage, check doses or replace your nurse line. Families see your warning signs and your number, and call you.
Leave your email and practice name and we'll be in touch. Or email hello@sbros.com.
